Social Worker Degree Senior

The Social Worker - BSW Senior, reporting to the Manager or their designate, will coordinate care for patients, clients and residents. The incumbent will provide case management, education, advocacy, urgent consultations, referrals, and consultation to appropriate services and provides on-site and virtual social work services. Human Resources Exemption: No Experience - 2 years SW experience working with issues of capacity, dementia, grief, and loss - Two years experience with social work practice in a medical setting Education - Bachelor of Social Work (BSW) Competencies - Verbal communication skills - Written Communication Skills - Analytical skills Knowledge and Abilities - Sound judgment, tact, initiative and high standards of integrity/responsibility - Advanced Clinical Assessment Skills - Ability to conduct client assessments and engage in effective treatment planning - Ability to prioritize to meet deadlines and work effectively under pressure - Ability to perceive and analyze problems and effect innovative solutions - Ability to competently use computers and software applications - Knowledge in medical terminology - Ability to effectively counsel and work with individuals, families and groups - Ability to establish effective treatment relationships with clients - Competencies: Leadership Skills - Ability to work in a team setting in a postive manner - Ability to communi...
