The Assessor Coordinator Degree Senior is responsible for providing client assessment, care planning, advocacy, and coordination of services and monitoring of care needs. The Assessor Coordinator Degree Senior is responsible for the delivery of the home care program in their geographical area. The Assessor Coordinator Degree Senior will be assigned to provide a standard of care to clients that will meet the individual needs of clients in accordance with established professional standards, policies and procedures of the Saskatchewan Health Authority and in accordance with any governing body responsible for setting the guidelines for the professionals practice.
