Community Care Nurse (Term)

Reporting directly to the Health Manager, the Community Care Nurse is responsible for assisting in the implementation and evaluation of the home & community care, Adult In-Home Care, Diabetes, Chronic Disease Program, and Community Health Program. The Community Care Nurse delivers and assists with supporting clients in their homes during times of acute or chronic health issues. The Community Care Nurse also provides nursing care to clients of the program such as catheter insertion, wound care, foot care, and nursing assessment. The Community Care Nurse works with family members and community resources to ensure Elders and others with chronic health conditions are cared for safely and respectfully
